Program Eligibility Requirements

Financial Assistance

To receive support from this fund, a referral must come from a social service worker with appropriate documentation of the need. The Lex-Care coordinator and at times the Board of Directors, review the request. Following appropriate approval the referral is featured in the column. No one is featured in the column without his or her written permission.

Diaper Fund

Families in need of an emergency supply of diapers may contact the following agencies: Cardinal Valley Center, Salvation Army, and Gainesway Community Empowerment Center. A referral from a social worker is not necessary. Families are required to provide identification, social security card, and verification of the child’s birth date.

Rides Program

Clients may be referred to Lex-Care RIDES for up to $50.00 worth of trips per year.  A client must not have alternative means of transportation.  Transportation is provided inside Fayette County.  All transportation (Bus, taxi or Wheels ) assistance needs to be approved before the transportation service is rendered. Referrals must be made through a social worker.

General Eligibility Requirements

Anyone in need is eligible. This is not limited to Fayette County residents, since the Herald-Leader circulates all over the state. All referrals must come from an agency, church staff member, or volunteer who is a Lex-Care member. The client may not refer him/herself. All clients must sign a release form before they can be featured. The Board of Directors has set the maximum assistance at $750.00 over a five year period for any one client. Lex-Care will not provide assistance for medications. If you would like to discuss the appropriateness of a situation before making a referral please contact the coordinator at GetHelp@LexCare.org.
