Referral Process


How to make a Financial Assistance Referral

You will need to obtain a referral packet. Referral packets are available on the Forms page, by calling or writing the office, or e-mailing us. Referring agents fill out the intake form. Please provide as much detail as possible. Please be sure to have your client sign the release form. In addition to the application and release form, please include any supporting documentation (proof of income and bill to be paid). If your client is in his / her current situation because of a medical condition, please enclose a medical professional’s statement.

Completed referral packets may be scanned and e-mailed or faxed to 859-422-5993. If the referral is selected to be featured in the "Make A Difference" column, the release form will also need to be forwarded to us before the client case may be featured. When will financial assistance be provided?

Lex-Care will send payment to the listed vendor when funds are available for the financial assistance request. Vendors will be contacted to verify the need prior to sending a check. Assistance is sent directly to the vendor, not the applicant. We will inform the person making the referral of any assistance provided. The process to receive assistance from Lex-Care can take up to four weeks, so please keep that in mind when making a referral. For more information on financial assistance go to our Forms page.

Referrals must be made through a social service agent. Please review the eligibility criteria form before filling out the request forms. All forms are located on the Forms page.
